Passport Office Open Sunday, May 20, 2007 for a special Passport Family Fair @ City of Lake Forest City Hall

Are you and your family traveling out of the country this summer? If you need a passport, the Lake Forest Park Passport Office at City Hall will be open on Sunday, May 20, from 11 am to 3 pm to complete the passport application process.

What do you need to bring?

Requirements for Passport:

Certified Birth Certificate or Certificate of Naturalization (no Hospital or Birth Abstract Certificate.
All applicants must appear in person (Adults and Minors)
Both parents must appear with Minors under the age of 14 yr old
Proof of Identification
Two current photo (current or taken within last 6 months)
Social Security number for all applicants
Application DS-11 available on site or on City of Lake Forest Park web site.

Adults $97		Minors (under 16) $67

Passport photos are available at City Hall check or cash for $16

The Lake Forest Park Passport Office is open Monday through Friday noon to 6 pm and Saturdays 10 am to 5 pm.
